Confirm a suspicion?
Confirmation calls for a milk sample for bacteriological examination; it will make it possible to direct the choice of the antibiotic, but it is sometimes negative, the arthritis developing in a second time. If confirmation is essential, joint aspiration performed aseptically will show cloudy, rarely purulent joint fluid; it contains many leukocytes and a high level of protein. the presence of fibrin cells can make the collection unsuccessful. Bacteriological culture is not often successful.
Prognosis and treatment
The treatment must concern the mastitis, if it is still present, at the same time as the articular inflammation; in the absence of an antibiogram, the treatment of the udder is based on a probabilistic approach which leads to the choice of the antibiotic which has the best chance of reaching the bacterium. Local mastitis treatment is then combined with systemic treatment. do not forget that mycoplasmas are resistant to beta-lactams and sulfonamides.
Prevention
The prevention of these arthritis is based on the prevention of mastitis.
